An Army National Guard official says a “procedural error” led to the fatal crash of a UH-60 Blackhawk helicopter in a Mendon Field.

The helicopter crashed in January during a night training mission, killing three Warrant Officers: Steven Skoda, Christian Koch and Daniel Prial. 

Col. Richard Goldenberg said in a news conference that the crew had a flight control issue, and the procedures and action they took to correct it actually put the aircraft into a non-recoverable position. He said the specific maneuver the crew was flying is now being restricted only to flight simulators. 

Col Goldenberg says the condition of the aircrfaft and the airframe at the time of takeoff was normal.

Voice recordings of the crew were recovered and used in the investigation, but will not be made public.
